  2. The advisor will ask for your identification, National Insurance number, and bank account details. Provide the information you have prepared.

  4. The advisor will confirm your application details and may request additional documents if necessary.

Mail Application:

  1. Download or receive the application form by mail.

  3. Attach copies of your identification, medical records, and any other required documents. Always send copies, not originals.
